PLASMA THERAPY OR PASSIVE IMMUNIZATION

General introduction

Plasma :- plasma is the liquid, cell free part of blood. it is liquid matrix of blood, it is about 55% of your blood.

plasma is yellowish in color due to the presence of pigment called as bilirubin.

Composition

it contains about 90% water.

solid part :- 8 to 10%  

1.proteins :- 6 to 7%  

A. Globulin :- produce and secreted by liver and lymphoid organ.

In blood 3 types of globulin present.

alpha globulin

beta globulin

Gama globulin - it is produced by lymphoid organs. it is present in the form of antibodies which destroy bacteria, virus & toxic substances. Also called as immunoglobulins.

Gama globulin is of 5 types

IgA

IgM

IgD

IgE

B. Albumin

C. Prothrombin :- Help in blood clotting.

D. Fibronogen :- Help in blood clotting.

NOTE:- PLASMA WITHOUT CLOTTING FACTORS IS CALLED AS SERUM.


Function 

Plasma is designed to carry nutrients, hormones, and proteins to the different parts of the body.
it also carries away the waste products of cell.





1.What is plasma therapy?

FATHER OF PASSIVE IMMUNIZATION :- Von Behring 'He discovered the process of passive immunization'.

He prepared the antidiphtherial serum (ADS) by injecting antigen into sheep.

Plasma therapy or passive  immunization is only possible due to the self healing property of our body.
When any pathogen (disease causing agent) enters in our body our immune system get activates and producing a army of antibodies. These antibodies binds with pathogens and destroy them.
Some peoples have strong immune system their immune system do it fastly .
But some have weak and their immune system do not produce sufficient antibodies in require time to fight with that pathogen.

In passive immunization we give ready made antibodies to the patients so that these antibodies fight with the pathogen and destroyed it.
It is the plasma which contains antibodies so we also called it plasma therapy.

We take plasma from recovered patients for a specific pathogen and then give it to another affected patients(infect by same pathogen) whose immune system  can't able to produce them Fastly.

Active immunization  is just opposite  to it. In active immunization  we don't give ready made antibodies to heal .
We use vaccines (contain the weak form of disease causing agent ) to developed immunity inside the person for Any specific pathogen. 
When next time that particular pathogen enters in the body our immune system recognize it an acts fastly to kill that pathogen. 
This is also called secondary response or anamnestic response.

In the case of snake bite we need fast antibodies production which our body can not able to do.  so, we use ready made antibodies which make through horses. because, horses can produce antibodies against venom and do not die easily on snake bites.

You know that after snake bite within 2-3 hour the person dies.
So we need fast action to destroy that pathogenic substances (venom). 
In this case we don't  wait for the response of the patient own immune system to produce antibodies.
So, we give ready made antibodies also called as anti-serum. 
The injections which is given to the patient contained performed antibodies against the snake venom.

Anti serum is plasma without clotting factor  but contains (immunoglobulins) 
To fight with specific pathogen.

In the case of covid-19 we also use convalescent plasma therapy to treat the patient. 


2.How plasma is separated OR collected  from blood ?

plasma, which is used to make life saving plasma protein therapies, is collected by a  process known as plasmapheresis. During this process, an automated device is used to separate the plasma from the blood.
Once the plasma is collected, red blood cells and other components are returned to the donor. After the plasma is collected, it is tested for suitability for further manufacture, frozen, and then held for 60 days prior to pooling(mixing all plasma collected from different donors).
it must pass these rigorous steps to be pooled with other plasma donations.
each plasma pool is further tested before the protein extraction process begins.

During this process, life saving protein are extracted from the plasma.
The separated, specific proteins are purified and adjust for optimal dosage before becoming the finished therapeutic product.Each source plasma donation is tracked the entire time.

3.How it is helpful in treatment?

Convalescent plasma is the liquid portion of blood gathered from someone who has recovered from a particular disease. Some promote using convalescent plasma in treatment as it may contain antibodies that can help a recipient to better fend off an infection.

Convalescent plasma has been investigated in previous outbreaks of respiratory infections, including with the SARS epidemic in 2003, the H1N1 virus between 2009 and 2010, the MERS outbreak in 2012 and the Ebola virus in West Africa in 2014.

4.Is it final  treatment ?

It is not the final treatment for pandemics until we are not vaccinated and develop immunity against the pathogen.
But, For many disease like tetanus, diphtheria, botulism we give plasma therapy to cure. 
This process not applicable for all diseases due to some limitation.

So, the question to this answer is obviously yes but the condition is that it should be hygienic.

In summer you observe that many vendors sell sugarcane juice at road side. Preparing it instantly and many people’s consume it they think that it is freshly prepared and also pure thereafter they felt sick (vomiting and diarrhoea) is common illness caused after drinking unhygienic juice.

The reason to felt sick after drinking the sugar cane juice from road side vendors is the poor hygiene maintenance.




As you know that flies always seeking around the sugarcane when the sugarcane crushes between rollers and your fresh juice is preparing and  some time the greedy flies also crushed between the rollers  and mixed into that juice and makes it unhygienic or unhealthy.

Definition:-

Cancer is defined as the uncontrolled , abnormal , excessive cell divisions anywhere in the body.   

                                            or

A malignant tumour formed anywhere in the body by abnormal , uncontrolled, excessive cell growth in any part of the body.

Lets see that what is the basic difference in normal cells and cancerous cells.



The growth of normal cell of our body is regulated by genes or we can say the cell division is in under control of  regulatory genes .

Types of tumours:- Malignant and Benign or difference between malignant and benign tumour

Benign tumour :- A benign tumour is the tumour formed in anywhere in the body which is non cancerous and formed by the normal cell of body.

    non-fatal
2. Growth is slow and limited (under control of genes).
3. No angiogenesis.

Malignant tumour:- Malignant tumour is made up of cancerous cells.

Types of cancer 

There are more over 100 types of cancer,which affect different parts of body.basically the classification of cancer is depend upon the organ and cell type where abnormal cell division begin. for example- cancer that begin in lungs is called as lung cancer.

 A. Carcinomas :-Cancer of skin cells , epithelial tissues, gland such type of cancer arises in epithelial lining of internal organs.


Carcinoma is originated when the epithelial cell divides uncontrollably.The layer of epithelial cell present almost in all body organs as a protective or covering layer.


So,carcinoma is further categorize on the basis of where it grows in body. for example if it begin in epithelial cell layer of lungs it is called as lung cancer.

6. Adenocarcinoma (gland tumour)
7. Gastric cancer

Sarcomas or Melanocarcinoma:- these are cancer growths of melanocytes(a type of skin cells).



B. Sarcomas :- these are derived from the tissues of mesodermal origin, ex- bones, fat and cartilage.
they are rarely found in human(about 1% of all tumors).

Some examples are :-

1. Bone cancer           - osteosarcoma 
2. Muscle cancer        - myosarcoma
3. Lymph node cancer - lymphosarcoma

C. Leukaemias and Lymphomas :- 

These are the tumors of hematopoietic cells. These are also called as blood cancer.


The blood formation is occur in bone marrow. when the blood forming cells inside bone marrow grows rapidly and form tumour (malignant) then it is blood cancer or leukaemias. 


1.Chronic myelogenous leukaemia (CML) :- This fatal cancer occurs mainly due to reciprocal translocation between chromosomes-22 and chromosome-9 which produce abnormal chromosome 22 (philadelphia chromosome).

2.Burkitt's lymphoma :- This produce due to reciprocal translocation between chromosome-8 and chromosome-14.


D.Glioma :-  cancer of glial cells (it arises in brain and spinal cord).



E. Germinoma - Cancer of gametes forming cells is called as germinoma. cancer of testes and ovaries.


Etiology of cancer

There are three genes which regulates normal cell growth in our body.
When these gene alter due to some reason(carcinogen).
Alteration in these genes leads to oncogenic transformation.


Genes 

a. Proto-oncogene
b. Tumour suppressor gene -inhibit cell proliferation.
c. Suicide gene -programmed cell death.

All these are involved in normal cell growth.
cancer is caused by mutant alleles of these genes, whose products do not respond to normal regulatory signals.as a result, the mutated cell proliferate uncontrollably.

Cancer cell do not show contact inhibition ( when cell comes in contact they inhibit uncontrolled growth by activation of tumour suppressor gene).









Carcinogens 


Transformation of normal cells into cancerous neoplastic cells may be include by physical, chemical, or biological agents.these agents are called as carcinogens.

1.Radiations :- Ionising radiations like x rays and gamma rays and non-ionising radiations like uv rays cause damage to DNA leading to oncogenic transformation.


2.Chemical factors:- Chemical carcinogens present in tobacco smoke have been identified as a major source of lung cancer.

Heavy smoking n-nitrosodimethylamine causes oral cancer , cancer of larynx and lungs.

Combustion of coal and pesticides , artificial flavouring agents, sweeteners, synthetic food, cause hormonal imbalance in body and it leads to cancer.

3.Physical factors :- Sharp teeth can cause tongue cancer.

Kashmiri peoples keep ''kangri" close to skin that causes skin cancer or ''kangri cancer''. 

4.Biological factors:- Cancer causing viruses called oncogenic viruses have genes called viral oncogenes.


                       Diagnosis of cancer



Early detection of cancer is essential as it allows to be treated successfully in many cases.

There are some technique for cancer detection and diagnosis:-

Blood examination


Increased wbc count in case of leukaemias.
detection of tumour markers like alpha fetoproteins (AFP) for liver cancer.

Biopsy

A piece of suspected tissue or tumour is cut into very thin pieces, stained and is examined under microscope.this is done by pathologists. 

In this pathologist study the structure of cell cancer cell have very high karyoplasmic index, nucleus large and irregular, nucleolus large, no. of mitochondria and ribosomes increased.

F.N.A.C.

Fine needle aspiration cytology eg- breast cyst/tumour. Fluid is collected from tumours and examined for presence of cancer cells.

PAP SMEAR 

It is used for cervical cancer diagnosis. slide is prepared from cervical fluid.

X-ray, C-T scan and MRI

These techniques are used to diagnosis of cancer of internal organs such as brain.


TREATMENT OF CANCERS



Surgery 
By removing entire cancerous tissue or tumour.

Radiotherapy

Tumour cell are killed by the exposure of radiation by taking proper care of normal cells.

Chemotherapy 

In this chemical  are  used to treat cancer or chemotherapeutic drugs are used to kill cancer cells.
Anti cancer drugs inhibits the cancerous cell growth.
